If Sherlock had somehow obtained a US-style exacto knife, I doubt John would recognise it as such - he'd probably just think "craft knife". The equivalent (slightly different, but just as standard & widespread) boxcutter type craft knife would be a stanley knife.

He (and so, I presume, she) would say "chocolate bar" or perhaps "snack". Sherlock is punctilious about correct use of English to the point that 'The Great Game' gave us that opening scene with the hung/hanged joke, so s/he wouldn't use "gotten" - it's grammatically incorrect in UK English. Sherlock would use a construction such as "you will have grown hungry" or "you will have become hungry."
On a non-Brit picking note (might as well be hanged for a sheep as a lamb) are you quite sure you want Mary to suggest Sherlock's homosexual, rather than lesbian, or gay? I mean, fair enough, if you feel strongly about that word choice - as a reader, I tend to think of it as implying masculinity rather than as gender-neutral (probably because of the homo=man thing, subconsciously) so when Sherlock was revealed to be a woman it felt particularly "...the who with the what now?" But that might just be me.
...God, I feel like a complete git now. Argh.
